Ambulatory NEMT Service Rates
(Private Pay & Medicaid-Authorized Options & Worker Compensation)
One-Way Base Rates
- 0-5 miles: $35
- 6-15 miles: $55
- 16-30 miles: $75
- 31-50 miles: $100
- 51+ miles: $3.50 per mile (pre-approval required)
Round Trip Flat Rates
- 0-5 miles: $70
- 6-15 miles: $95
- 16-30 miles: $135
- 31-50 miles: $175
- 51+ miles: $3.25 per mile each way (pre-approval required)
Wait Time Charges
- First 15 minutes included
- After 15 minutes: $25 per hour
After-Hours, Weekend & Holiday Rates
- Additional $30 surcharge applies to rides after 6:00 PM, weekends, or holidays
Monthly Subscription Plans
- Basic Plan: $140/week for 3 round trips (Mon/Wed/Fri)
- Flex Plan: $180/week for 5 round trips (M-F)
- Premium Plan: $625/month for up to 22 round trips…
